Lifetime Health Cover – Statement of Movements Clarification

The information in this circular supersedes the information contained in PHI Circular 29/07, issued on 23 May 2007, related to DIAC Form 424B.

Department of Immigration and Citizenship (DIAC) Form 1359

In some circumstances, an insurer may need proof that a person has been out of Australia in order to correctly calculate their LHC loading. These circumstances relate to a person who is an Australian citizen or permanent resident who:
  • was out of the country when LHC was introduced on 1 July 2000; or
  • was out of the country on their LHC base day; or
  • has been overseas for a period of greater than 12 months.
The appropriate documentary evidence in these circumstances is a statement of movements in and out of Australia. Such a statement is available from the Department of Immigration and Citizenship (DIAC). Individuals can access their records directly from DIAC at the counter or by completing the appropriate form - Form 1359 “Request for international movement records”. This form can be downloaded from the DIAC website.
