There are three lines representing medical, surgical and other separations. “Surcharge changes” is marked on each line at October 2008. 
Medical separations commence at slightly above 70%; surgical separations commence at approximately 20%; and other separations commence at slightly less than 10%. The lines continue essentially constant across all months.
