This survey has been developed as part of the Evaluation of the National Suicide Prevention Program (NSPP). It covers NSPP funded activity from 2006-2013.

The aim of the survey is to capture additional data to supplement the information obtained from progress/final reports and external evaluations data. This survey also gives project coordinators the opportunity to reflect on and recount some of the key issues and milestones encountered in the implementation of their NSPP-funded project(s).

If your organisation has NSPP funding for more than one project, you will need to submit a separate survey for each individual project. If your organisation received funding from multiple sources, please respond with information about the NSPP funded component of your project only.

Please note that the comments you make in this survey will remain confidential and that your organisation will not be identified in any reports related to this survey.
